American Geriatric Care Management

Free in-home Assessment

Our team begins to gather vital information about the senior’s needs for home care. We ask questions to get an overall picture of the senior’s situation including contact information, illnesses or diagnoses, current medications, living conditions, and mobility issues. This helps us determine the level of care a senior needs.

Care Plan Development

Our team will identify and help clarify the senior’s unique needs — physical, emotional, spiritual or social. It might also include instructions for how they will manage, and how they wish to be treated, and in the event of an emergency situation.

Caregiver Matching

Once the family approves the care plan, we look for caregivers with similar interests and we staff foreign language assignments when possible. We guarantee our matchmaking abilities. If the senior or the family is not comfortable with the fit, we change the caregiver immediately for one more suitable to your needs and comfort level.

Ongoing Supervision and Communication

Our team provides supervision and random inspections to ensure safe, competent care and to stress the importance of punctuality, reliability, and quality of home care. All of our caregivers record the care they provide to ensure that all duties are performed and all home care needs are met. Care notes can include general observations and services delivered.
